From the moment of placement: here’s what happens.

A foster child’s placement can be stressful. YouthCare is here to offer help for members and foster families—right from the start.

At placement

Members are enrolled into YouthCare right when they’re placed. Each member gets a temporary Member ID that can be used to access care right away.

The Member ID and other information is also immediately available in the Member Portal.

An initial health screening will be done by the Illinois Department of Children and Family Services (DCFS). A DCFS caseworker or investigator will go over the results of this screening with the member and the foster parents or caregiver following placement. The screening will also be submitted to a HealthWorks agency for review.

Within a few weeks

Each member will receive a welcome packet with a permanent Member ID card, Member Handbook, and more. The packet should arrive within 7 to 10 business days. Foster parents and caregivers can also expect emails and text messages to help them get the most out of YouthCare.

Each member will also be assigned a GuidedCare Care Coordinator. Care Coordinators help members connect to the care and support they need.

Within a month

The member’s DCFS caseworker will make sure a Comprehensive Health Evaluation (CHE) is done within 21 days.

YouthCare’s Member Outreach Team will also have members complete a Personal Wellness Assessment during the first 30 days. The assessment information will be given to the member’s Care Coordinator to help develop a plan of care.

Note: It’s important to complete screenings and assessments on time. This helps make sure the right care and services will be available to the member.
