Personal support for every member’s unique needs
GuidedCare Care Coordination helps all members get the care they need to live healthier. Every YouthCare member is assigned a Care Coordinator who provides one-on-one support. Care Coordinators work to understand members’ health needs. Then they partner with caregivers and providers to connect members to the right care and resources.
Care Coordinators can be messaged directly through the Member Portal.
How Care Coordinators support members
Care Coordinators begin working for members from the start, helping complete the Personal Wellness Assessment to fully understand the member’s health and any concerns.
Throughout the care journey, Care Coordinators:
- Spend time getting to know the member and their unique needs
- Make a care plan to meet healthcare goals—including mental health
- Help you understand and use YouthCare benefits
- Answer questions about treatments
- Assist with referrals to specialists
- Connect you with resources and support in your community
Your Care Coordinator is here to provide support and answers. They’ll check in with you regularly. That may be monthly, quarterly, or at other intervals. Timing depends on the level of care the member needs.
