COMMUNITY BEHAVIORAL HEALTH PROVIDERS ASKED TO COMPLETE PATHWAYS TO SUCCESS SURVEY

The Illinois Department of Healthcare and Family Services (HFS), in partnership with the HealthChoice Illinois managed care organizations (MCOs) and the Illinois Association of Medicaid Health Plans is releasing a survey to gather feedback from community mental health providers about Pathways to Success services and supports that HFS is implementing throughout the state.
Pathways to Success provides access to enhanced care coordination and other behavioral health services and supports for Medicaid enrolled youth under the age of 21 years who meet the Department’s Behavioral Health Decision Support Model.
HFS is working to further develop a provider network for the following Pathways to Success Services: Intensive Home-Based, Family Peer Support, Therapeutic Mentoring and Respite. HFS would like direct feedback from community behavioral health providers regarding their level of knowledge about these services and their interest in offering these services.
HFS and our contracted MCOs are requesting that providers complete the Pathways to Success Provider Survey by September 15, 2023.
