For personalized support
Contact the member’s GuidedCare Care Coordinator. The contact information can be found in the Member Portal.
To talk with a registered nurse
Call the 24/7 Nurse Advice Care Line at 844-289-2264 (TTY: 711) if you can’t reach the member’s provider. Use this number when you have health questions or you’re not sure what kind of care might be needed.
For mental health crisis support
Call the 24/7 Crisis and Referral Entry Services (CARES) line at 800-345-9049 (TTY: 773-523-4504)) for help. If it’s an emergency, call 911 or go to the nearest emergency room. You can also call the Suicide and Crisis Lifeline at 988.

To schedule a ride
Call the member’s Care Coordinator or call Member Services at 844-289-2264 (TTY: 711). You can also book a ride using the MTM app.
To get a digital Member ID card
Download a digital card from the Member Portal. It can be used right away. (To get a new card mailed to you, reach out to Member Services.)
