Use your card to connect with care
Each member receives their own YouthCare Member ID card. It’s the key to getting care. The member or caregiver should always carry the card and present it whenever receiving care services or picking up medications.
The card has phone numbers to call with questions and requests for services and support. There’s also information that’s specific to the member, like the name and number of their primary care provider.
For providers, there’s information that’s required to deliver care services and process claims with YouthCare.
