DCFS Former Youth in Care Eligibility
Included populations:
- All youth who were previously in the custody of the Department of Children and Family Services (DCFS) including youth who have been adopted, are living with kinship providers, have returned to biological parents, and/or youth who have left the DCFS system.
- DCFS Former Youth in Care receiving the following Home and Community Based Waiver Services are included in managed care enrollment
- Persons with Disabilities waiver
- Persons with HIV or AIDS waiver
- Persons with Brain Injuries waiver
Excluded populations:
- For the DCFS Former Youth in Care population, the following Home and Community Waiver Services are not considered for managed care enrollment
- Persons who are elderly (Aging waiver)
- Persons on the Supported Living Program waiver
- Persons receiving services from the Developmental Disabilities waiver (age 0-18)
- Children on the Medically Fragile Technology Dependent (MFTD) waiver
- Children approved for the Nursing and Personal Care Services (NPCS) program
- Third Party (Private Insurance)
- Youth who are on their parents’ insurance, as long as Health and Family Services (HFS) recognizes this insurance to be comprehensive. Parents are to report third party insurance coverage in one of the following ways:
- Call the Third Party Liability (TPL) Inquiry Line at 217-524-2490 and speak with a Customer Service Representative. The representative will help you add or update private insurance information.
- Fax your request to add or change the TPL unit at 217-557-1174.
- Scan and send your request to add or change information by email to HFS.TPL.1442@illinois.gov.
- Youth who are on their parents’ insurance, as long as Health and Family Services (HFS) recognizes this insurance to be comprehensive. Parents are to report third party insurance coverage in one of the following ways:
